  • A Settlement has been reached with General Physician, P.C. (“GPPC”), the Defendant in a class action lawsuit about a cybersecurity incident that occurred between April 6, 2024 and June 12, 2024, and resulted in the potential unauthorized access to or acquisition of people’s Private Information (the “Data Incident”).
     
  • You are included in this Settlement as a Settlement Class Member if you received a letter from Defendant notifying you that your Private Information (i.e. full name, address, Social Security Number, financial account information, date of birth, medical history information, mental and physical treatment information, diagnosis information, treating physician, medical record number, health insurance information, policy number, subscriber number, or group/plan number) may have been accessed and/or acquired by an unauthorized party as a result of the Data Incident.
     
  • As a Settlement Class Member, your rights are affected whether you act or do not act. Please read this Notice carefully.
     
YOUR LEGAL RIGHTS AND OPTIONS IN THIS SETTLEMENTDEADLINE
SUBMIT A CLAIM FORMThe only way to receive cash and other benefits from this Settlement is by submitting a timely and Valid Claim.

OPT OUT OF THE SETTLEMENTYou can choose to opt out of the Settlement. This option allows you to sue, continue to sue, or be part of another lawsuit against the Defendant and Released Parties, as defined in the Settlement Agreement, related to the legal claims resolved by this Settlement. You can elect to retain your own legal counsel at your own expense. If you opt-out, you will not be able to receive any Settlement Class Member Benefits and you will not be bound by the terms of the Settlement Agreement. The Settlement Agreement can be found on the Documents page of this Settlement Website.

OBJECT TO THE
SETTLEMENT AND/OR ATTEND A HEARING
 
If you do not opt out of the Settlement, you may object to it by writing to the Court about why you don’t like the Settlement. You may also ask the Court for permission to speak about your objection at the Final Approval Hearing, scheduled to be held on June 4, 2026. If you object, you may also file a Claim for Settlement Class Member Benefits.
